New ‘Health and Wellbeing Service’ Opens In West Sussex

Launching from mid-July onwards, the Health and Wellbeing Service is a social prescribing service, working in partnership with GP practices across West Sussex. Southdown already successfully provides this Service across East Sussex and Brighton and Hove and Sussex Partnership NHS Foundation Trust.
The service will support people aged 18 or over who have a diagnosis of bipolar disorder, schizophrenia and psychosis, to improve their health and wellbeing following their free annual Physical Health Check (PHC) appointment. We are also temporarily offering support for people to attend their PHC.
Southdown’s Mental Health & Wellbeing Advisors will work specifically with people following their Physical Health Check to support them to develop personalised and practical solutions to everyday issues. We will help people to explore services, groups and activities that support them to improve their health and emotional wellbeing.
Our Health & Wellbeing Advisors will offer face-to-face and remote support in a range of settings from GP practices, to community venues across West Sussex

“We are excited to be able to roll out this service to the West Sussex area and will build upon the excellent work already achieved in the Brighton and Hove and East Sussex service. The service is an exciting and important step towards reducing health inequalities across the Sussex area for those who have been diagnosed with severe mental illness. We will work alongside Sussex Partnership NHS Foundation Trust, GP practices and the Primary Care Networks to target those who need support. We will encourage and guide people to access the brilliant services in their local areas. We are expanding our team and welcoming dedicated staff to deliver this service working closely with other Southdown services linking in with Social Prescribers and Community Connectors within Southdown and looking forward to working closely with providers and partners in the West Sussex area”
