Discharge to Assess
About the service
The Discharge to Assess service in Brighton and Hove offers a short-term package of tailored support and accommodation to people ready to leave mental health inpatient settings. The service is delivered by Southdown in partnership with Brighton and Hove City Council and Sussex Partnership NHS Foundation Trust.
People who no longer need the higher levels of support and supervision provided on the ward, are offered a six-week tailored package of one-to-one support to transition from their inpatient setting, back into their own accommodation and the community. Where needed, Southdown will also provide temporary self-contained, furnished accommodation to support the client’s transition.
Discharge to Assess provides support and advocacy with mental health recovery, to sustain accommodation, improve daily living skills and rebuild connections in the wider community. Personalised support is provided on weekly basis.
We work alongside clients and mental health services to personalise support that enables clients to manage their mental health, as well as identify and address other issues in order to improve clients’ happiness and wellbeing.
We have a dedicated Housing Specialist based within Mill View hospital in Hove, who supports Brighton and Hove clients to secure or retain suitable accommodation in preparation for their discharge.
Eligibility
This service is available to adults aged 18 and over, who live in Brighton & Hove and are either currently on an acute mental health ward, or at risk of admission.
How to access
All referrals for this service must come from Sussex Partnership NHS Foundation Trust staff.
Referrals will need to have a lead practitioner and a move-on plan to be eligible for support.
When considering whether our service is suitable for someone, please bear in mind that we offer medium to low support needs for people that are able to live independently. Our staff are not based on site at the accommodation and cannot administer medication.
